MISSION STATEMENT:

The Outagamie Caregiver Coalition is a network of organizations dedicated to assisting people with identification as caregivers and providing needed support through comprehensive education and resources. 

PRESENTATION LIST: The Outagamie Caregiver Coalition is excited and proud to share this wealth of resources, education, and enlightenment with you.  There are many professionals in or associated with the Caregiver Coalition who are experts in their area of practice and are happy to share that wisdom with you or your group or organization.

CAREGIVER SUPPORT GROUPS:

‣ Caregiver Chats (Outagamie County Caregiver Coalition): Guided, Themed Casual Chats where family caregiver participants share and learn from each other.  This group meets the 4th Wednesday of the month at 4pm.  This is a ZOOM Virtual Meeting; For information, contact Karen Dickrell, UW Extension, (920) 832-4763. ‣ Men's Dementia Support Group: For men who are caring for and/or have transitioned their spouse into a care facility.  The group is held the 3rd Thursday of each month at 10:00am.  Location: Elite Office Suites, 3701 E. ‣ Mosaic Family Caregiver Group, in Partnership with Outagamie County Aging Unit: For Family/Friend Caregivers. CAREGIVER TRAINING & EDUCATION:

‣ Powerful Tools for Caregivers: 6 week, evidence-based training designed to provide family caregivers with tools to care for themselves while caring for a loved one.  DATES to be announced.  This is currently a ZOOM Virtual Meeting; Contact Carrie at 920-483-0898 for information or to register

‣ Mosaic RCI REACH Program, in Partnership with Outagamie County Aging Unit: Evidence-based, personalized, in-home, 1:1 caregiver training program.  DATES & LOCATION: Individually scheduled; Will be set up between trainer and Caregiver/participant.  Ask for: Heather Flick, Susan Kazik or Jackie Gonzalez at 920-832-2783
