The Outagamie Caregiver Coalition is a network of organizations dedicated to assisting people with identification as caregivers and providing needed support through comprehensive education and resources. 

PRESENTATION LIST: The Outagamie Caregiver Coalition is excited and proud to share this wealth of resources, education, and enlightenment with you.  There are many professionals in or associated with the Caregiver Coalition who are experts in their area of practice and are happy to share that wisdom with you or your group or organization.

Share the Care:

Share the Care is a process that helps individuals and families broaden their circle of support by engaging family members, friends, co-workers, church members and others in helping to support an individual as they age or during and illness.

Prepare to Care:

This programs helps individuals and families develop a care plan for themselves and/or for a loved one.  The caregiving plan is based on the needs and wishes of those who will be receiving the care.  It includes a 5-step plan and workbook.

Caregiver Cafe': Making Time for You:

People come away finding support, new ideas, and resources that will directly help them as they care for their loved ones.

Thompson Center Caregiver Support Group: for spouses and adult children of individuals affected by Dementia or Alzheimer memory loss.  Support with a friendly group format and educational guidance. New members welcome.  The group is held on the Last Wednesday of the month from 9:30-11:30 am. at the Thompson Center.   An activity group for the individual with memory loss is held on site at the same time by the Fox Valley Memory Project.   Thompson Center, 820 W College Ave, Appleton, WI.  Call Lutheran Social Services at the Thompson Center at (920) 225-1704 for more information.

Caregiver Support Group: For anyone touched by Alzheimer's Disease or memory disorder.  This group meets the second Monday of the month from 4-5 pm. at St. Paul Elder Services, 316 E. 14th St. Kaukauna, WI  Call 766-6186 for information.

Spouse Support Group: for spouses of individuals affected by memory loss with a group format of education and support.  The group is held on the 4th Monday of the month from 10-11:30 am.  Care is available in our Club Gabriel Adult Day Services Program upon request for spouses in need, however those arrangements need to be made in advance. Young Onset Support Group: for individuals under the age of 65 who have been diagnosed with Alzheimer's Disease or other memory disorders.  Issues unique to this group will be addressed and discussed in both an educational and support group format.  The group is held on the first Monday of the month from 9:30-10:30 am at St. Paul Elder Services, 316 E. 14th St. Kaukauna, WI  Call 766-6186 for information.
